  Consular Outreach Service

Once again the Philippine Consulate General in Chicago is coming to the Filipino Cultural Center (FCC) to provide consular services during a special 2-day session on October 6 from 8 AM-7 PM and October 7 from 8 AM-12 PM. Available services will include:

• Passport Services (renewal, replacement of lost passport, amendment/extension of passport)
• Dual Citizenship
• Visa Services
• Legal and Notarial Services (special power of attorney, authentication, etc.)
• Civil Registry (report of marriage, report of birth, etc.)

Read below carefully for details and costs!

A. For Passport Applications

1. Applicants must register prior to the event to get a scheduled appointment slot. No walk-ins accepted. To register e-mail chicagopcg.outreach@gmail.com with a copy of your passport (for renewal) or birth certificate (for first-time application).

2. Please indicate the following information in your email: (a) complete name; (b) date of birth; (c) place of birth; (d) sex; (e) civil status; (f) citizenship; (g) location of consular outreach.

3. Acceptance of appointment requests will be limited to 250 applications on a first-come, first-served basis. Requests for appointment will be granted through e-mail notification in the order in which they are received.

4. Applications should be submitted at the venue during the service. Applicants are required to present all supporting documents (originals and photocopies). Applications with missing requirements will not be entertained and processed. You are advised to visit the Consulate's official website to ensure you have all the requirements.

B. For Dual-Citizenship Applications

1. Applications must be submitted to the Philippine Consulate General (122 S Michigan Ave, Suite 1600, Chicago, IL 60603) for pre-processing. Indicate on the face of the mailing envelope the phrase "Consular Service in Kansas."

2. Deadline for the submission of applications/petitions is September 26.

3. Applicants are advised to check the requirements and the downloadable petition or application form at the Consulate's official website. Except for the completed application form and the photograph, applicants may submit just photocopies of the requirements along with the application form to the Consulate. However, applicants are advised to bring the originals at the consular venue for presentation to the processing officer.

4. Applicants will be served in the order the applications are received by the consulate. You will be informed of your appointment time by e-mail.

C. Applications for Other Services

1. Applications for civil registry (e.g. report of marriage and report of birth) and notarials (e.g. special power of attorney, authentication, certification, etc.) should be submitted during the service. Applicants are required to present the supporting documents (originals and photocopies) with their applications.

2. Applicants do not have to get an appointment. They will be served on a first-come, first served basis.

D. Order of Activities

The consular services will start at 8 AM. There will be a program from 9-9:30 AM, where representatives from the consulate general's partner agencies, such as the Occupational Safety And Health Administration (OSHA), National Labor Relations Board (NLRB), and Equal Employment Opportunity Commission (EEOC), may participate.
